One Hackney referral Criteria and Form

Who is One Hackney for? One Hackney is an integrated service for people with complex needs. People who are at high risk due to being frail and who may be vulnerable in a number of ways including being in debt, having problems coping alone, living in inappropriate housing, and being lonely and isolated. This may lead them to make inappropriate use of services including Accident and Emergency or general practice. These are just some examples; there will be a whole range of needs and individuals requiring support.

Who can refer into One Hackney?  Anyone. If you are concerned about someone please do refer them.

 How do we refer?

 By telephone (please have the information available we ask for on the referral form)

  • Fax referral form to the Care Coordinators (0207 729 8303)

  • Email referral form to the appropriate Quadrant Care Coordinator
